I'm a little late to the party (11 months), but ...
Kevin Carson makes a tremendous case that Government Transportation Subsidies are a substantial portion of the cause of the shape of our economy. Winner take all markets are (substantially) the reason we have a super-rich 1%...and the reason we have winner-take-all markets (outside of software) is that we have massive transportation subsidies...and have since ~1850.

"The 747 is essentially a spinoff of military production."

No state subsidy of airlines, no 9/11.
Just in case anybody thought these subsidies don't murder people.
The truck and rail subsidies murder too, it's just less obvious.

Second, this is one of the reasons a sudden shift to anarchy is a bad idea. The large scale transportation networks have replaced what would actually be efficient. If government suddenly interrupted its actions, the large networks would collapse but the smaller networks wouldn't have time to re-form before people started starving to death.
